One way, the price is for the whole car, not per person. The ferry is not included in the price: plus ferry tickets, about 80 THB per person.
What one price covers
The fare is for the car, not for each passenger. The driver, fuel, expressway tolls and taxes are inside it, and so is the wait at your hotel for the first few minutes. The traffic on the road does not change it.
From 24 December to 5 January 400 THB is added per car.
Dividing the car
A Standard Sedan with three people works out at a third of 3,899 THB each. The Toyota Innova at 4,099 THB takes four with their cases, and the Toyota Fortuner costs 4,199 THB. For six people the Hyundai H1 is 6,099 THB, and the Toyota Commuter takes nine at 4,799 THB.
One traveller pays the full car price, which is the case where a bus is cheaper. A bus ticket costs far less per person. What the car adds is the trip from door to door, the luggage on the seat beside you and a departure time of your choice.
Against the bus
Buses to Trat leave from the Eastern bus terminal in Bangkok, and from Trat a songthaew or minivan goes to the ferry pier. The ticket is far cheaper for one person, but you travel to the terminal first, change at the end, and pay for the ferry and the island transport on top. The steps are on the how-to-get page.
The taxi collects you at the hotel and drives onto the ferry with you. Time is the real difference.

The ferry is not included in the price: plus ferry tickets, about 80 THB per person. The price also leaves out personal extras such as tips.
